Outreach Services

Outreach programs are designed to help individuals with mental and physical disabilities remain living in their home. Easter Seals trained caregivers use therapeutic intervention to assist children and adults with daily living and socialization skills. Caregivers also assist with personal and home care.

Easter Seals provides many services to people in the comfort of their homes.  Many families with loved ones in need of care use home health workers in a variety of ways.  We supply in-home workers to meet our clients’ needs.  All of our in-home workers are screened, trained, drug tested and have BCI Checks (finger prints for criminal record).

Services provided

Personal Care Services- Provides assistance with bathing, toileting, dressing, ambulating, transferring, eating and any other ADL’s (activities of daily living).

Homemaker Services- Provides assistance with housecleaning, laundry, essential shopping, errands, and meal preparation.

Respite Services- Provides a sitting service designed to visit with a client while the family gets a much needed break.

We work with the case managers to deliver quality care and work on individual goals. Funding for in-home care comes from the Area Agency on Aging- Passport, IIIE and Title IIIB, Trumbull County Senior Service Levy, ODJFS waiver, MRDD waiver and private pay. Click here for additional information.
